Exchange mailbox in Outlook delay

Some of the users on our network are getting an odd issue that I'm struggling to find a solution to, so I'm wondering if anybody has any suggestions.

At random users mailboxes don't seem to be updating in Outlook. When the user then logs into Outlook Web Access they find a batch of unread e-mails that haven't come up in their inbox in Outlook. Most often the delay is around half an hour but on a small number of occasions it's been hours. The occasions that it has been hours I have found that the user has cached Exchange mode activated and I have put it down to that and disabled it, but I can't be sure. On most of the instances of a half hour delay cached exchange mode wasn't selected.

This is only an issue that started coming up since we have moved most users from Windows XP to Windows 7, and to my knowledge all of the users that have had the issue are on 7. This could just be co-incidental due to the small number of XP machines we have remaining. The clients version of Outlook has included 2002, 2003, and 2010. Again my initial thoughts were that it was due to still using older versions of Office, until it started happening on PC's running Office 2010. Yes it's all updated.

The mail server is a HP Proliant G5 ML350. It's on Windows Server 2003 Std x64 and Exchange 2007. It's completely updated and restarted. We had a few errors through the logs in relation to certificates having expired, which I have now resolved and three mailboxes not inheriting permissions (a knock-on effect from the exchange 2000 upgrade I suspect). Other than those there are no errors in any of the system logs.

Edit: Selecting Send/Receive has not updated the inbox in any of the instances this issue has been found (Outlook XP, 2003, or 2010). Windows firewall is disabled by group policy, and we're running Kaspersky Endpoint Security 8 with application control disabled.

The users that have had the issue are mostly heavy users that deal with a lot of attachments although all are well within mailbox limits, but we have had the issue on 2003 with a reasonable e-mail user on basic e-mail as well so I don't think it's e-mail/mailbox size related.

Any suggestions on where to go next with this one?
